29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. (a) On or about March 22, 2017, the employer had not ensured that all Powered Industrial Truck Operators were trained according to this standard, in that their training only consisted of a Video Presentation. There was no performance evaluation conducted and certified by the employer.

29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut: (a) On or about March 22, 2017, in the Carpenters Barn of the Lodge facility an employee engaged in routine carpenter cutting activities was operating a table saw without the blade guard attachment. As a result, the employee received amputation injuries to the fingers on his left hand.
