29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use. On March 28, 2017, located at the northwest exterior wall of a four-story commercial structure, an employee was working inside a jobsite-design personnel work platform attached to the carriage back rest of a rough terrain forklift. The personnel work platform was not approved by the manufacturer for use with this equipment. Potential fall heights were greater than 9' above the concrete surface below.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied. On March 28, 2017, located at the northwest exterior wall of a four-story commercial structure, a rough terrain forklift was being used to support an employee working inside a scaffold platform (jobsite-design personnel work platform attached to the forklift carriage back rest) that was not attached to the fork(s). Potential fall heights were measured to be greater than 9 feet.
