29 CFR 1910.107(e)(9): Flammable liquids for use in spraying operations were transferred from one container to another without both containers being effectively bonded and grounded to prevent discharge sparks of static electricity.  On or about March 21, 2017  at the rear of the building, the employer was allowing employees to transfer a flammable liquid 3 (Resin) with a flashpoint of 84 degrees Fahrenheit. Employees were transferring Resin flammable liquid from a 55 gallon drum that was not grounded into a plastic/general bucket or container to mix with dry products.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.  On or about March 21, 2017 the employer was allowing employees to use an electrical extension cord in the center of the warehouse ceiling, connected to a floor fan with the strain relief pulling away from the receptacle plug.
