29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about March 29, 2017 at a worksite located at 18545 Blosser Road, Dalton, OH 44618, during framing activities including roofing work, the employer did not ensure employees were protected from eye hazards by ensuring eye protection was available and worn while operating pneumatic nail guns at approximately 120 psi and circular saws.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about March 29, 2017, at a worksite located at 18545 Blosser Road, Dalton, OH 44618 , during framing activities at a height greater than six feet, the employer did not ensure employees were protected from fall hazards of approximately 11 to 17 feet by ensuring a fall protection system/device was being used.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  On or about March 29, 2017, at a worksite located at 18545 Blosser Road, Dalton, OH 44618, employees were exposed to fall hazards of approximately 11 feet while using an extension ladder to assess the roof.  The ladder did not extend at least 3 feet above the landing area and there was no grasping device to assist employees in mounting and dismounting the ladder.
