29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: On or about March 29, 2017 at the Paul Mueller Company facility- employees were exposed to a struck by hazard from a Dayton Bench Grinder, in the Maintenance Shop, which had tongue guards that were improperly adjusted in excess of one-fourth inch.
