29 CFR 1926.100(a):     Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    At the commercial construction site located at 8620 Blue Ridge Blvd in Kansas City, Missouri: Employees engaged in masonry activities were exposed to head injury from falling tools or materials.  Employees engaged in ground work activities around and under scaffolding were not provided with hard hats.

29 CFR 1926.451(e)(1):    When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:     At the commercial construction site located at 8620 Blue Ridge in Kansas city, Missouri: Employees were exposed to a fall hazard. The supported scaffold was not provided with an access ladder for the upper working level. Employees were exposed to a fall hazard of up to approximately 14 feet 3 inches while climbing the scaffold frames.

29 CFR 1926.451(g)(4)(i):    Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold was released for use by employees other than erection/dismantling crews.      At the commercial construction site located at 15525 Switzer Road in Overland Park, Kansas: Employees engaged in masonry activities were exposed to a fall hazard of approximately 14 feet 3 inches from the upper working level of the scaffold to the ground below. A complete guardrail system or other means of fall protection was not provided.

29 CFR 1926.451(h)(1):      Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contained or deflected the falling objects:       At the residential construction site located at 8620 Blue Ridge Blvd in Kansas City, Missouri: Employees were exposed to a struck-by hazard. Masons were exposed to falling material and tools while working on lower levels of 4 section tubular framed scaffold. Toe boards were not provided.
