29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded:  Instances:  a) On or about 3/31/17, at the above address jobsite second production room, the  grinding machine tool & cutter (SN 315AQ15Z-0062) was not guarded at the point of operation.   b) On or about 3/31/17, at the above address jobsite at the entrance of the second production room, the Hardinge lathe was not guarded at the point of operation.  c) On or about 3/31/17, at the above address jobsite at the rear of the second production room, the Hardinge lathe was not guarded at the point of operation.  d) On or about 3/31/17, at the above address jobsite at the exterior of the building, the HW Acheon Machinery lathe was not guarded at the point of operation.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  Instances: 1) On or about 3/31/2017, at the above address jobsite, the employer had not replaced the cover for live parts on the CNC machine (LB1511) while its was in use by the operator at the second production room.   2) On or about 3/31/2017, at the above address jobsite, the electrical receptacle outlet had live parts exposed and a cover was not installed properly while it was in use at the second production room.

The spray bottle was used by several employees while working in the machine shop.
