29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a. On or about 03/24/2017, for employees roofing a low slope roof, in that, holes were surrounded by a warning line system. An employee fell through one of the holes 39 feet 9 inches to the ground below inside the building.
