29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. L3 Technologies, Inc. Located at 7900 Block Telegraph Road. Gate 7816 Fort Belvoir, VA 22080: 03-03-2017 on or about 9:45 A.M. , an employee suffered a broken right femur after a fall resulting in a formal in-patient hospitalization admission to the hospital. The employer did not report the in-patient hospitalization to the Occupational Safety & Health Administration until 03-09-2017 at 8:00 P.M. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
