29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  On or about March 21, 2017, Fixins Home Services, as the controlling contractor, did not ensure employees of Alpha Exteriors, LLC and Antonio Pacheco were protected from fall hazards, while conducting reroofing activities, without the benefit of fall protection systems being in place, on a roof in excess of six feet above the lower surface.    Fixins Home Services, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 992978, Citation Number 1, Item Number 1, and was affirmed as a final order on October 7, 2014, with respect to a workplace located at 825 Midland Ave., Manitou Springs, CO.    Fixins Home Services, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 997745, Citation Number 1, Item Number 1, and was affirmed as a final order on November 6, 2014, with respect to a workplace located at 232 East Fontenaro Street, Colorado Springs, CO.
