29 CFR 1926.502(b)(10): Each employee engaged in roofing activities on low-slop roofs, with unprotected sides and edges 6 feet (1.8m) or more above lower levels was not protected from falling by a guardrail system, safety net system, personal fall arrest system, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. (a) On or about March 23, 2017, and at times prior thereto, employees were observed working on a flat roof probing the seams on a rubber membrane roof without a fall protection system in place. Note: Abatement certification IS required for this item.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about March 23, 2017, and at times prior thereto, employees involved in roofing activities utilized an extension ladder to access a flat roof that extended only 1 1/2 foot over the parapet wall. Note: Abatement certification IS NOT required for this item
