29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  a)  Swaad Palace Restaurant - On or about March 1, 2018, the employer did not provide abatement certification for citation 1 Item 1, 29 CFR 1910.151(c) issued on 09/25/2017 with a final order date of 10/23/2017. In which abatement was due by 10/20/2017.  b)  Swaad Palace Restaurant - On or about March 1, 2018, the employer did not provide abatement certification for citation 1 Item 2 29 CFR 1910.1200(e)(1) issued on 09/25/2017 with a final order date of 10/23/2017. In which abatement was due by 10/20/2017.  c)  Swaad Palace Restaurant - On or about March 1, 2018, the employer did not provide abatement certification for citation 1 Item 3, 29 CFR 1910.1200(g)(8) issued on 09/25/2017 with a final order date of 10/23/2017. 29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  (a) Workplace -  On or about April 3, 2017, the employer did not have an eyewash station where employees are required to use corrosive materials such as Ridgway's Oven and Grill Cleaner and Elite Professional Bleach.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:  (a) Workplace -  On or about April 3, 2017, the employer had not developed or implemented a written hazard communication program for employees that were required to use bleach and other corrosive chemicals, including but not limited to Blast-Away Oven and Grill Cleaner to clean.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
