29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) This violation occurred on April 3, 2017 when three employees working on the ground level, without wearing head protection, was exposed to head injuries while passing a sheets of plywood to employees working on an upper level.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) This violation occurred on April 3, 2017 when employees were exposed to a fall hazard of approximately 24 feet while roofing a townhouse. Israel Concino WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(13) WICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1126365, CITATION NUMBER 1 ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON April 19, 2016, WITH RESPECT TO A WORKPALCE LOCATED AT 7031 Isle Royale Ln. Irving, TX 75063.
