29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  On or about March 30, 2017, at the above addressed jobsite, employees were operating a suspension scaffold, for which the scaffold and scaffold components had not been inspected for visible defects by a Competent Person before each work shift, and wherein the scaffold and scaffold components were observed to have several safety deficiencies, therefore exposing the employees to a fall hazard.  NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration; such activities were not performed only by experienced and trained employees selected for such work by the competent person.  On or about March 30, 2017, at the above addressed jobsite, employees were operating a suspension scaffold, for which the scaffold and scaffold components had not been erected, moved, dismantled and altered by trained and experienced employees, while under the supervision and direction of a Competent Person who was qualified in scaffold erection, moving, dismantling or alteration and wherein the scaffold and scaffold components were observed to have several safety deficiencies, therefore exposing the employees to a fall hazard.  NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about March 30, 2017, at the above addressed jobsite, employees were working from a suspended scaffold while wearing a PFAS, during which the system was rigged such that during a fall, the employees would free fall more than 6 feet and/or the system was rigged such that the employee would have contacted the lower level, therefore exposing the employees to fall hazards.   NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  On or about March 30, 2017, at the above addressed jobsite, employees were using a Personal Fall Arrest System and guardrail system, while working on/from a suspended scaffold, during which the employer had not provided the employees with training by a Competent Person who was qualified; in the correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used; and/or in the use and operation of guardrail systems, personal fall arrest systems, and other protection to be used; and/or did not verify compliance with paragraph (a) of this section by preparing a written certification record of training, therefore exposing the employees to fall hazards.   NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
