29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about April 3, 2017, the employer did not ensure that employees working on underground boring were provided with training necessary for hazard recognition and safe work practice. This condition exposed employees to caught between hazards. Employee's right hand was crushed while retrieving boring pipe out of the machine.
