29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  On or about March 29, 2017:  a) Employees were exposed to eye injuries from flying particles presented by the use of pneumatic nail guns and hammers, when the employer failed to ensure that each employee had and was using proper eye protection.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about March 29, 2017:  a) Employees installing floor sheathing on a new residential structure were exposed to fall hazards of 8.5 feet to the outside of the structure, when the employer failed to provide and ensure the use of fall protection measures were being used.  b) Employees installing floor sheathing on a new residential structure were exposed to fall hazards of 10.9 feet to the basement concrete slab below, when the employer failed to provide and ensure the use of fall protection measures were being used.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about March 29, 2017:  a) Employees installing floor sheathing on a new residential structure were exposed to fall hazards of 8.5 up to 10.9 feet, when the employer failed to properly train the employees to recognize and minimize the fall hazard(s) present.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
