29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse. Employees engaged in storing and moving pallets of dog food in the warehouse were exposed to struck by hazards in that pallets stacked four high, approximately thirteen feet were not secured against sliding and collapse.

General-duty citation text
1910.178(m)(3) Unauthorized personnel shall not be permitted to ride on powered industrial trucks. A safe place to ride shall be provided where riding of trucks is authorized. Employees engaged in inventory within the warehouse were exposed to fall hazards in that employees were being raised up on the forks of powered industrial truck approximately six to nine feet to read labels on pallets when conducting inventory.
