29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a forklift. a) Throughout: On or about April 4, 2017, and at times prior, the employer had not ensured that each employee operating the Crown Pallet Jack PE4500 Type E Series SN 6A322360, Crown Forklift 5200 Series Type E SN 9A186461, and Crown Pallet Jack PE 4500 Type E SN 6A332815 had completed powered industrial truck training. Aleva Trucking and Produce was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, which was contained in OSHA inspection number 1181467, citation number 1, item number 2 and was affirmed as a final order on January 19, 2017 with respect to a workplace located at 150 Forest Parkway, Forest Park, GA 30297.
