29 CFR 1910.212(b): Machines designed for a fixed location were not securely anchored to prevent walking or moving.    A. Multilink, Inc. located at 580 Ternes Ave., Elyria, Ohio: On or about April 4, 2017, the employer did not ensure the vertical drill presses were secured to the floor to prevent them from moving or tipping over. Nine vertical drill presses (6 Rigid & 3 Craftsman) used in the Star Fighter area were not secured to prevent movement exposing employees to tip over hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.     A. Multilink, Inc. located at 580 Ternes Ave., Elyria, Ohio: On or about December 1, 2016, the employer did not ensure employees were protected while operating the Bridgeport Drill Press located in the Star Fighter Area. An employee sustained serious injuries while using the drill press exposed to the rotating chuck and drill bit. The Bridgeport drill press chuck and bit were not guarded exposing employees to serious hazards.     B. Multilink, Inc. located at 580 Ternes Ave., Elyria, Ohio: On or about December 1, 2016, the employer did not ensure employees were protected while operating the six Rigid and three Craftsman Drill Presses located in the Star Fighter Area. Employees using the drill presses were exposed to the rotating chuck and drill bit. The chuck and bit areas on the drill presses were not guarded exposing employees to serious hazards.

A. Multilink, Inc. located at 580 Ternes Ave., Elyria, Ohio: On or about December 1, 2016, an employee sustained serious injuries resulting in hospitalization. The injury information was not recorded on the 2016 OSHA 300 Log until April 4, 2017.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA.    A. Multilink, Inc. located at 580 Ternes Ave., Elyria, Ohio: On or about December 1, 2016, the employer failed to report an employee hospitalization (admittance) following a worksite incident.  The employer or employer representative failed to contact the Toledo Area Office or the 24 hour OSHA number to notify the agency of a employees hospitalization within 24 hours of admittance. The injured employee was admitted on December 1, 2016, for related treatment.  The employer failed to contact OSHA between the time of admittance and the complaint received by the Toledo Are Office.
