29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units:    On or about April 5, 2017, at the above addressed jobsite, employees were working from a tubular welded scaffold up to approximately 12 feet high without all platforms being fully planked, exposing the employees to fall hazards.       Instance (1): 2nd tier of the scaffold on the back of the townhome not fully planked.    Instance (2): 1st tier of the scaffold on the back of the townhome not fully planked.    Instance (3): Side of the townhome 2nd tier not fully planked.

Crossbraces were used as a means of access:    On or about April 5, 2017, at the above addressed jobsite, employees were working on a tubular welded scaffold up to approximately 16 feet high without the employer providing an access ladder, exposing the employees to fall hazards.
