29 CFR 1910.179(b)(8): Only designated personnel shall be permitted to operate a crane covered by this section:    Manufacturing building, welding areas, plow bay - on or about April 5, 2017, the employer failed to designate employees who were adequately trained in safe operation and load handling to move steel parts overhead bridge cranes 1 (East) and 2 (West). Welders were exposed to hazards associated with falling loads from below-the-hook lifting devices likely to result is serious injury. 29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle:    (a) Manufacturing building, AccurPress EQ-4000 250 ton press brake - on or about April 5, 2017, an employee was assigned to hand-feed, align, and support small metal parts for single and multiple bends in the unguarded point of operation. Operator's fingers and hands were exposed to the pinch point created during the operating cycle.    (b) Pacific Hyraulic Press Brake serial no. 900377 - on or about April 5, 2017, an employee was assigned to hand-feed, align, and support small metal parts for single and multiple bends in the unguarded point of operation. Operator's fingers and hands were exposed to the pinch point created during the operating cycle.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.254(d)(1): Workmen assigned to operate or maintain arc welding equipment were not acquainted with the requirements of 29 CFR 1910.254(d) and 29 CFR 1910.252(a), (b), and (c)    Manufacturing building, welding areas - on or about April 5, 2017, employees assigned to operate arc welding equipment to fabricate steel parts did not receive instructions and qualifications in its safe operation and use as required by 1910.254(d), and were not acquainted with the requirements of 1910.252(a), (b), and (c); including, fire protection and prevention, protection of personnel, and health protection and ventilation. Welders were exposed to hazards associated with unsafe use of welding equipment likely to result in serious injury/illness.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.179(e)(2)(i): A crane shall be provided with bumpers or other automatic means providing equivalent effect, unless the crane travels at a slow rate of speed and has a faster deceleration rate due to the use of sleeve bearings, or is not operated near the ends of bridge and trolley travel, or is restricted to a limited distance by the nature of the crane operation and there is no hazard of striking any object in this limited distance, or is used in similar operating conditions. The bumpers shall be capable of stopping the crane (not including the lifted load) at an average rate of deceleration not to exceed 3 ft/s/s when traveling in either direction at 20 percent of the rated load speed:    Manufacturing building, welding areas, plow bay, 2 ton overhead bridge cranes 1 (East) and 2 (West) - on or about April 5, 2017, bumpers, stops, and/or other equivalent means were not installed on runways and/or crane bridges to prevent accidental contact or to reduce impact where two moving cranes could strike each other during travel on the same runway. Welders were exposed to hazards associated with cranes components' failures upon overhead contact that could affect safe operation and load handling.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.179(j)(2): Frequent inspection. The following items 29 CFR 1910.179(j)(2)(i) to 29 CFR 1910.179(j)(2)(vii) shall be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard:  Manufacturing building, welding areas, plow bay, 2 ton overhead bridge cranes 1 (East) and 2 (West) - on or about April 5, 2017, required frequent daily to monthly inspections were not performed for:  - maladjustments interfering with proper operation and excessive wear of all functional operating mechanisms,  - deterioration and leakage of hydraulic or air system lines, tanks, valves, drain pumps, and other parts,  - hooks deformation and cracks, - hoist chains excessive wear, twist, links distortion interfering with proper function, and stretch beyond manufacturer's recommendations,  - rope reeving noncompliance with manufacturer's recommendations, - below-the-hook lifting devices conditions affecting safe load handling.  Welders were exposed to hazards associated with components' failures affecting safe operation and load handling.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not required to wear appropriate goggles, to protect them from the rays when noncombustible or flameproof screens or shields were not used:  Manufacturing building, welding areas - on or about April 5, 2017, flameproof screens were not provided and used to enclose the perimeter of welding activity. Employees working in areas adjacent to welding activities and walking by such areas were exposed to arc welding rays that may cause damage to their eyes and skin.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
