29 CFR 1910.134(h)(3)(iii): Self-contained breathing apparatus were not inspected monthly, maintained in a fully charged state, recharged when the pressure falls to 90% and the employer did not determine that the regulator and warning devices function properly. Employees engaged in well servicing operations were exposed to inhalation and asphyxiation hazards from the SCBA system not functioning properly.
