29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: a) On or about 4/11/17 - Mixing room, employee(s) were provided a 3M 8511 face filtering respirator for voluntary use when weighing and mixing sugar floss components. On 4/11/17, during floss mixing, an employee's exposure to Particulates Not Otherwise Regulated (Total Dust) exceeded the Permissible Exposure Limit of 15mg/m3. The employer had not identified and estimated the employee's exposure to PNOR (Total Dust), required the use of the respirator.
