29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    (a)  On or about April 5, 2017 in the Production Area, the Kuraki CNC Horizontal Boring/Milling Machine (AKB-13) table traversed on the x-axis by the aisle way exposing employees to struck by hazards.    (b)  On or about April 5, 2017, in the Production Area, the Kuraki CNC Horizontal Boring/Milling Machine (Model:  AKB-13, S/N:  30563) had doors, when opened during operation, would slow down the spindle speed exposing employees to moving parts.    (c)  On or about March 27, 2017, in the Production Area, the Haas VF (Model:  VF-1B, S/N:  37848) #207 had doors which could be opened during operation, loading, and unloading exposing employees to struck by hazards.     (d)  On or about April 5, 2017, in the Production Area, the Mitsubishi Vertical Machining Center #205 (Model M-V5CN, S/N:  v53334) had the door interlocks turned off allowing the doors to be opened during operations, loading, and unloading exposing employees to struck by hazards.    (e)  On or about April 5, 2017, in the Production Area, the Mitsubishi Machining Center #102 had the door interlocks turned off and the auto restart engaged allowing the doors to be opened during operations, loading, and unloading exposing employees to struck by hazards.    (f)  On or about April 5, 2017, in the Production Area, the Okuma CNC horizontal lathe #302 had the door interlock in the off position allowing the doors to be opened during operations, loading, and unloading exposing employees to struck by hazards.    (g)  On or about April 5, 2017, in the Production Area, the Makino Milling Machine (Model:  a71, S/N:  272) #105 had the Door Interlock Release on allowing the doors to be opened during operations, loading, and unloading exposing employees to struck by hazards.    (h)  On or about April 5, 2017, in the Production Area, the Makino Milling Machine (Model:  a81, S/N:  254) #108 had the Door Interlock Release on allowing the doors to be opened during operations, loading, and unloading exposing employees to struck by hazards.    (i)  On or about April 5, 2017, in the Production Area, the Mori Seiki (Model:  NH4000 DCG) #106 had the Door Interlock released allowing the doors to be opened during operations, loading, and unloading exposing employees to struck by hazards.    (j)  On or about April 5, 2017, in the Production Area, the Mori Seiki (Model:  NH4000 DCG, S/N:  NH401FL1531) #107 had the Door Interlock released allowing the doors to be opened during operations, loading, and unloading exposing employees to struck by hazards.    (k)  On or about April 5, 2017, in the Production Area, the Mori Seiki (Model:  GV-503) #203 had the Door Interlock released allowing the doors to be opened during operations, loading, and unloading exposing employees to struck by hazards.      (l)  On or about March 5, 2017, in the Production Area, the Fadal Vertical Machining Center (Model:  VMC 4020 HT, S/N:  032004117030) #206 had doors which could be opened during operation, loading, and unloading exposing employees to struck by hazards.     (m)  On or about May 11, 2017, in the Production Area, the Fadal Vertical Machining Center (Model:  VMC 4020, S/N:  012003055257) #213 had doors which could be opened during operation, loading, and unloading exposing employees to struck by hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    (a)  On or about April 5, 2017; the Hydra-Matic Machinery & Equipment (14917) "Big Blue" #401 had milling wheels which were unguarded exposing employees to caught in hazards and rotating parts at the point of operation.

29 CFR 1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.    (a)  On or about April 5, 2017, in the Production Area; the Ryobi 8" bench grinder had the work rest adjusted to an opening of approximately 3/4" exposing employees to struck by hazards.
