29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement. SOF Communication/Electrical Maintenance Facility Project site entrance: Employees worked next to unprotected rebar used to reinforce concrete formwork. Employees were exposed to impalement hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. Hallway of P1119 Construction Project site: Employees were working with a red and black striped flexible cord used to extend power to the wet vacuum cleaner which did not have strain relief. The insulation was pulled away from its outer sheathing. Employees were exposed to minor shock hazards.
