29 CFR 1910.38(a): The employer did not establish a written emergency evacuation plan for employees expected to evacuate in the event of a fire or other emergency:  At the establishment a written emergency action plan did not include procedures for emergency evacuation, procedures to account for all employee(s) following an evacuation, employee alarm systems, and name/job title of every employee who maybe contacted by employees who may need more information about the plan to inform all employees of the procedures to follow in case of fire or similar emergencies (tornadoes/thunderstorms/chemical spill/etc.), thus exposing employees to fire hazards.

29 CFR 1910.157(g)(1): Where the employer has provided portable fire extinguishers for employee use in the workplace, the employer shall also provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:  At the establishment, employee(s) allowed to use fire extinguishers to extinguish incipient stage fires were not provided with fire extinguishment training, thus exposing all employee(s) to fire hazards.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:  A work related incident which resulted in an employee hospitalization was not reported to OSHA within 24 hours.  The incident occurred on or about March 27, 2017 at 9:30 am and was reported to OSHA on March 30, 2016 at 10:00 am.
