29 CFR 1910.140(d)(3):The employer did not ensure that body belts were prohibited as part of a personal fall arrest system.    Warehouse Area- On or about April 6, 2017 operators of the Crown and Raymond Swing Trucks were exposed to fall from heights of approximately 12ft. while wearing a body belt as part of a Personal Fall Arrest System. Operators were likely to sustain internal injuries from falls at this height.       Abatement documentation and certification required for this item per 29 CFR 1903.19(d).

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in paragraph (l).      Production Areas- On or about April 6, 2017 employees were operating powered industrial trucks (Swing Trucks and Sit Down Forklifts) without being provided formal classroom training nor evaluation by persons who are qualified to do so. Employees in the area were exposed to caught between and struck by hazards.     Abatement documentation and certification required for this item per 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    Tesker Thread Rolling Machines- On or about April 6, 2017 machine operators were exposed to caught in hazards while placing hands between unguarded rolls/rollers to load and unload bolt blanks. No machine guarding was provided to protect operators from rotating components and the in-going nip point created.    Abatement documentation and certification required for this item per 29 CFR 1903.19(d).
