On or about 4/6/2017, the employer did not maintain the floors of the production area in a clean and dry condition.  There was an accumulation of water/coolant fluid on the floor in the Haven Machine ad Mill number 7/tubing machine areas and employees were exposed to slip, trip and fall hazards.
