Location: 62-68 69th St., Guttenberg, NJ.  a) Employer, excavation subcontractor, while conducting excavation operations; did not provide protection against cave-ins at the open excavation thereby exposing employees at the site to caught in and struck by hazards.  The employer allowed other trades men to work in an open excavation that was over 15 feet deep with nearly perpendicular walls in Class B soil and had no cave-in protection or adequate benches / slopes to protect the employees from engulfment/injury; on or about 3/23/17.
