Location: 62-68 69th St., Guttenberg, NJ. a) Employer, foundation subcontractor, while conducting foundation operations; did not provide safe means of egress in an open excavation greater than 5 ft. in depth that lacked protection against cave-ins thereby exposing employees at the site to caught in and struck by hazards; on or about 3/23/17.

Location: 62-68 69th St., Guttenberg, NJ. a) Employer did not provide protection against cave-ins at an open excavation approximately 15 feet deep with nearly perpendicular walls in Class B soil, thereby exposing employees at the site to caught in and struck by hazards, on or about 3/23/17.
