29 CFR 1910.1020(d)(1)(ii): Each employee exposure record was not preserved and maintained for at least thirty years: South Process Area and Adjacent: On or about April 4, 2017 and at times prior thereto, employee exposure records did not reflect exposures of employees required to perform semi-annual shutter and three year leak test surveys on level controllers of the LP Flare Scrubber and NGL Separator. This condition underestimated employee exposure to ionizing radiation sources.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: TG Regeneration Skid: On or about April 4, 2017 and at times prior thereto, during the process of adding an 85% Triethanolamine solution to balance the ph of the process, the placement of the facility for flushing the eyes was not provided in the work area in accordance with the standard. This condition exposed employees to eye irritation hazards.
