29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: ScanVision Rig Drill Site: On or about April 5, 20217, the shaft guard was missing from the charging pump on mud pump #2. The power shaft is located between the centrifugal pump and transmission. This condition exposed the employee to an amputation hazard.

The condition exposed the employee(s) to electrical shock hazards.
