29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about March 31, 2017, the employer did not ensure the operator of a Sheldon, Model 15 lathe machine was protected from the rotating parts of the chuck and work piece, by implementing effective machine guarding.

29 CFR 1910.157(g)(2): The employer shall provide the education required in paragraph (g)(1) of this section upon initial employment and at least annually thereafter. On or about April 7, 2017, the employer did not provide employees with training upon their initial assignment on the procedures and use of portable fire extinguishers, or in lieu of the use of portable fire extinguishers trained employees on a fire prevention/emergency evacuation program.
