29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition: Cactus Drill Rig #133 Site: On or about April 4, 2017, the metal panel to the rig floor where the flexible cables channel runs through was not level. The panel was lifted approximately 2 3/4 inches above the floor. This condition exposed the employees to trip hazards.
