29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision: Savanna Rig 413 Drill Site: On or about April 6, 2017; the employer failed to have an electrician reattach a front cover to an AC unit voltage panel which exposed live electrical parts to employee contact. This condition exposed the employees to electrical shock hazards.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Savanna Rig #413 Drill Site: On or about April 6, 2017; the employer did not properly ground the trailers to earth in the following instances. This condition exposed employees to electrical shock hazards. a) Transformer power supply to Rig Manager's trailer and crew trailer. b) Bonded trailers: 1000 gal diesel tank, Lube Skid, Generator trailer, Hydraulic power trailer, Mud Pump house #1 and Mud Pump house #2.

29 CFR 1910.305(a)(2)(xi): Cable assemblies and flexible cords and cables were not supported in place at intervals that ensure that they will be protected from physical damage: Savanna Rig #413 Drill Site: on or about April 6, 2017, the 480 volt overhead wires running between the engine trailer and carrier trailer were left hanging low. This condition exposed employees to struck-by hazards.
