29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501   a.  On or about April 7, 2017, at the above addressed jobsite, employees performing residential roofing operations on a steep sloped roof (4:12 or greater), seventeen (17') feet seven (7") above the ground, were not protected from falls, thereby exposing employees to fall hazards.

On or about April 7, 2017, at the above addressed jobsite, employees carrying roofing material ascending the portable ladder used to access the roof did not maintain three points of contact, thereby exposing employees to the hazards associated with falls.
