29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:   The employer contracted the services of HF Rubber Machinery, Inc. for the installation of the #17 TSR.   The employer and HF Rubber Machinery, Inc. did not inform each other of their respective hazardous energy control procedures.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group:    On or about April 5, 2017, Continental Tire North America, Inc., did not ensure that the employees of its subcontractor HF Rubber Machinery, Inc. conducted group lock out tag out prior to engaging in the installation of the #17 TSR.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence:  a)  Employees were exposed to machine hazards associated with the uncontrolled electrical, pneumatic and gravitational energy when employees were engaged in service and maintenance activities on the #17 Inclined Batch Off Conveyor.  The employer failed to implement energy control application steps and ensure each authorized employee had the knowledge of the type, magnitude, the hazards, and the method or means to control energy prior to performing service work [per the 1910.147(d)(1)].  As a result, the remaining applicable energy control elements, involving equipment shutdown [(d)(2)], machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine service hazards.   b) Employees were exposed to machine hazards associated with the uncontrolled electrical and pneumatic energy when employees were engaged in service and maintenance activities on the #17 TSR.  The employer failed to implement energy control application steps as residual energy was not dissipated [per the 1910.147(d)(5)(i)].  As a result, the remaining applicable energy control elements, verification of isolation [(d)(6)], was not implemented to protect employees from machine service hazards.      The Continental Tire North America, Inc. was previously cited for a violation of this occupational safety and health standard 1910.147(d), which was contained in OSHA inspection number 1050581, citation number 1, item number 1a and was affirmed as a final order on February 8, 2016, with respect to a workplace located at 11525 North IL Hwy 142, Mount Vernon, IL.
