Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about 4/7/17, at the above addressed jobsite, employees were exposed to a fall hazard while installing roof tiles on a roof of a church due to lack of a fall protection system in place.

PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
