29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) On April 4, 2017, at the Hantover Underground Storage Warehouse, employees were exposed to serious injuries to the eyes and body, in that the employer had not provided adequate PPE to include face shield, gloves and apron to employees engaged in Forklift Battery Maintenance Activities.

(a) On April 4, 2017, at the Hantover Underground Storage Warehouse, the employer did not ensure that all Power Industrial Truck Operators were trained and evaluated, in that one operator employed with company since 2014 had not completed any of the required training.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: (a) On April 4, 2017, at the Hantover Underground Storage Warehouse, the employer did not ensure that all Power Industrial Trucks in need of repair had been taken out of service, in that one of the Toyota PITs was missing a safety attachment pin for the lifting forks. As a result, employees were exposed to struck-by injuries in the event of a load or fork blade drop during operations.
