On or about April 6, 2017, on the L&J presses, the point of operation guards that are installed had fasteners (wing nuts and pins) that are readily removable by the operator, thereby exposing the employees to amputation hazards.

On or About April 6, 2017, on the L&J presses serial numbers 52001 and 51966 , the point of operation of the presses were not fully guarded, thereby exposing employees to amputation hazards.     The Meyer Steel Drum, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.217(c)(2)(I)(a), which was contained in OSHA inspection number 1140927, citation number 1, item number 7 and was affirmed as a final order on August 11, 2016, with respect to a workplace located at 2000 S. Kilbourn, Chicago, IL 60623.
