29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   At the jobsite, employees operating Paslode nail guns while performing roofing activities were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.    At the jobsite, employees engaged in roofing activities at a height of over 6 feet were not protected by a fall arrest system. Employees were exposed to an unprotected fall of over 20 feet to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access;    At the jobsite, Employees engaged in roofing work were exposed to fall hazards while using a ladder that did not at least three feet above the roof's landing.
