29 CFR 1910.212(a)(1):  One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about April 7, 2017, one or more methods of machine guarding were not provided to protect operators and other employees in machine areas of the LZR Fit building:    a)  Polisher employees operating the Head Polisher were exposed to the ingoing nip point between the head pulley and sanding belt of the Head Polisher.    b)  Polisher employees operating the Hand Polisher were exposed rotating parts between the work piece and the rotating chuck of the Hand Polisher.    c)  Polisher employees operating the Liner Polisher were exposed to ingoing nip points. Ingoing nips points included but were not limited to; between sanding belt and pulleys of the Liner Polisher attachment, and between the drive wheels of the Liner Polisher frame and the work piece cylinder.    d)  Machine Operators operating the C-Frame Press were exposed to caught-between hazards of the C-Frame Press. Caught-between hazards included but were not limited to; between the C-Frame's ram and the top of the work piece, and between the bottom of the work piece and the C-Frame's pedestal.    e)  Machine Operators operating the Boldrini Flanger were exposed to ingoing nip points. Ingoing nip points included but were not limited to; between the work piece and the forming dies, and between the work piece and the idler rollers of the Boldrini Flanger.    f)  Machine Operators operating the Hydro Press were exposed to caught-between hazards of the Hydro Press' die and ram. Operators initiated the press's cycle by a pendant which did not keep employees from being caught between the press's die and ram.    g)  Machine Operators operating the Faccin Flanger were exposed to ingoing nip points. Ingoing nip points included but were not limited to; between the work piece and the forming dies, and between the work piece and the idler rollers of the Faccin Flanger.    h)  Machine Operators operating the SMT Flanger were exposed to ingoing nip points. Ingoing nip points included but were not limited to; between the work piece and the forming dies, and between the work piece and the idler rollers of the SMT Flanger.    i)  Machine Operators operating the SMT Bump Press were exposed to caught between hazards and ingoing nip points. Caught-between hazards included but were not limited to between the frame of the press and the positioning table. Ingoing nip points included but were not limited to between the work piece and the drive wheels of the positioning table.    j)  Plasma Cutting Operators operating the Plasma Table were exposed to struck-by hazards from the control panel and carousel frame of the ALLtra Plasma Cutting Table.

The shafts were 2 inch in diameter and projected 3 inches.

29 CFR 1910.147(d):  The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  On or about April 7, 2017, employees were exposed to machine hazards associated with moving parts when they engaged in set-up activities on Faccin Bump Press. The employer did not ensure that employees implemented energy control application steps prior to allowing them to change die heads and clean rollers. The machine was not deenergized prior to employees engaging in service/maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.
