29 CFR 1910.333(a)(1): Live parts to which an employee was exposed were not deenergized before the employee worked on or near them: a) Costco Warehouse (Dairy Department): An employee working from the platform of a Genie scissor lift was required to perform electrical work on a ballast, rated for 440 volts, that was not deenergized. The employer did not ensure that the electrical system was deenergized prior to allowing the employee to perform work, thus exposing the employee to an electrical shock hazard, on or about, 4/10/17.

29 CFR 1910.333(b)(2)(i): The employer did not maintain a written copy of the procedures outlined in paragraph (b)(2) of 29 CFR 1910.333 and did not make it available for inspection by the Assistant Secretary of Labor and his or her authorized representatives: a) Costco Warehouse (Dairy Department): An employee working from a Genie scissor lift was required to perform electrical work on a ballast, rated for 440 volts, that was not deenergized. The employer did not maintain a written copy of procedures for the deenergization of the electrical system prior to allowing the employee to perform work, thus exposing the employee to an electrical shock hazard, on or about, 4/10/17.
