29 CFR 1910.9(b):     Training. Standards in this part requiring training on hazards and related matters, such as standards requiring that employees receive training or that the employer train employees, provide training to employees, or institute or implement a training program, impose a separate compliance duty with respect to each employee covered by the requirement. The employer must train each affected employee in the manner required by the standard, and each failure to train an employee may be considered a separate violation.  [FR 73 75583, Dec. 12, 2008]    a)  Training on operating the crane in CR-4 Center Bay East did not train operators on all hazards associated with inadequately clamping a coil with the Caldwell clamping device.

29 CFR 1910.179(n)(1): Size of load. The crane shall not be loaded beyond its rated load except for test purposes as provided in paragraph (k) of this section.      a) On 4/10/17 the crane in CR-4 Center Bay East was being used in excess of the load rating capacity, in that the coil load and Caldwell clamping below the hook lifting device were not added and/or calculated into the total weight being lifted, there by exceeding its lift capacity.    b) On or about May 3, 2017 a load of coil steel came into the facility that was approved by management for lifting and storage in the racking system that exceeded the load/lift capacity of the crane.
