29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures:  JOB SITE: 910 Fox Hill Road, Saint Charles, MO 63301, on or about 4/7/2017, three (3) workers were observed on top of a 3:12 pitch roof, approximately 9' 8" eave to ground level, actively removing old roofing material and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  JOB SITE: 910 Fox Hill Road, Saint Charles, MO 63301, on or about 4/7/2017, workers were observed using an aluminum extension ladder to climb up and down the roof, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  The workers were exposed to a fall hazard.
