29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Machine Shop - On April 13, 2017, the employer failed to ensure that procedures for the control of potentially hazardous energy were developed, documented, and utilized by machine operators when working on the Mitsubishi M-V70E Vertical Horizontal Boring and Turning machine, to perform tasks, including loading parts, cleaning chips/debris from the area and when performing measurements and validation of the program. Employees were exposed to machine hazards associated with moving and rotating parts.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    a) Machine Shop - On April 13, 2017, the employer did not ensure that employees working on the Mitsubishi M-V70E Vertical Horizontal Boring and Turning machine, that perform tasks, including loading parts, cleaning chips/debris from the area and when performing measurements and validation of the program, were trained as Authorized employees.  Employees were exposed to machine hazards associated with moving and rotating parts.      In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, rotating parts and flying chips:    a) Machine Shop - The employer failed to ensure that guards or methods of guarding on the Mitsubishi M-V70E vertical milling machine were adequate to prevent employees from making contact with the machine's point of operation, rotating parts, flying chips and a rotating table.  On April 13, 2017, an employee was observed performing set up and validation of the program when the safety interlocks on the machine's doors were disabled.     In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.
