29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about April 10, 2017 and times prior thereto, at the worksite located at 2401 South Arkansas Avenue in Russellville, Arkansas, employees on the second floor of the trimmer area were not protected from falling by the use of guardrail systems, safety nets, or personal fall arrest systems. This condition exposed employees to fall hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about April 7, 2017 and times prior thereto, at the worksite located at or near 2401 South Arkansas Avenue in Russellville, Arkansas, employee(s) working on the second story of the West Fraser building where a trimmer was being installed. The employees had not been trained on fall hazards, recognition, or how to minimize the hazards. This condition exposed employees to fall from height hazards.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l)    29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    On or about April 7, 2017 and times prior thereto, at the worksite located at or near 2401 South Arkansas Avenue in Russellville, Arkansas, employees were operating forklifts without any formal training or evaluation of the operator's performance. This condition exposed employees to struck by hazards.
