On or about 2/27/2017, employee engaged in picker operations, using a Ballymore, Grip 24"W 11 Step Steel Rolling Ladder to retrieve a 12 X 18 X 20 box of inventory from Q-Level, located approximately 10'4" from floor. Employee lost his balance and fell to the cement floor below.

On or about 2/27/2017, employee engaged in picker operations, using a Ballymore, Grip 24"W 11 Step Steel Rolling Ladder to retrieve a 12 X 18 X 20 box of inventory from Q-Level, located approximately 10'4" from floor. Employee lost his balance and fell to the cement floor below and was later hospitalized.
