29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a)  181 Ridgedale Avenue, Madison, NJ.    Workers were exposed to head injuries during rough framing work on this new residential home as no head protection was being used by any workers.                                              Violation observed on or about 03/23/2017.

29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  181 Ridgedale Avenue, On the Outside, Madison, NJ.    Workers involved in framing a new home were not using any method of fall protection and were exposed to falls of up to 20 feet to the ground below.                                            Violation observed on or about 04/11/2017.    b)  181 Ridgedale Avenue, On the Inside, Madison, NJ.    Workers were exposed to falls of 10 feet from the second to the first floor as the second floor hallway landing had no fall protection such as a guard rail system to protect workers.                                              Violation observed on or about 04/11/2017.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a)  181 Ridgedale Avenue, On the Inside, Madison, NJ.    Workers were exposed to falls of 10 feet from the second to the first floor as the portable extension ladder did not have its rails extend 3 feet above the upper landing surface when workers accessed the second floor.                                              Violation observed on or about 04/11/2017.
