29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: A) South side of job site, on or about April 9, 2017, an employee climbed a baker scaffold and then climbed a horizontal piece of metal attached to framing members to access the top of the metal decking, exposing the employee to a fall hazard of approximately 4 1/2 feet.
